Phase One Therapy accepts referrals for NDIS self or plan-managed participants, private paying clients, insurance and medicolegal clients.
Referrals can be submitted by clients, healthcare providers, support coordinators, or case managers, insurance agencies, employers and law firms via our website, email, or phone, including client details, funding source, and therapy needs.

Send Any Relevant Details
If you have any recent medical or Allied Health reports, please send to admin@stephaniem93.sg-host.com with any additional relevant information.
Our Team Will be in Touch
Once received, our team will review the referral and contact you to discuss your needs and schedule an initial appointment, either as a home visit or Telehealth session.
