Phase One Therapy may need to share the Client’s personal information to involve others in the Client’s healthcare. The following policy outlines the privacy practices of Phase One Therapy in regards to personal and business information and when, how and why it is shared.

Privacy

Phase One Therapy has adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act). The NPPs govern the way in which information is collected, used, disclosed, stored, secured and disposed of. These privacy laws regulate how personal information is obtained and handled, from the collection, it’s use and future disclosures as well as how the information is stored and when and how it is disposed. A copy of the Australian Privacy Principles may be obtained from the website of The Office of the Australian Information Commissioner at https://www.oaic.gov.au/.

Personal Information Collection.

Personal information is defined as any information that specifically identifies the Client or from which the Clent’s identity can be easily attained. Phase One Therapy collects Personal Information for the primary purpose of providing Allied Health services to the referred Client, and marketing. Phase One Therapy may also use the Client’s Personal Information for secondary purposes closely related to the primary purpose, in circumstances where the Client would reasonably expect such use or disclosure. The Client may unsubscribe from Phase One Therapy mailing/marketing lists at any time by contacting Phase One Therapy in writing.

When Personal Information is collected, Phase One Therapy will, where appropriate and where possible, explain to the Client why the information is being collected and how it is planning to be used.

Use and Disclosure of Personal Information.

The Client’s personal information will only be used to the extent that it informs Phase One Therapy staff and assists them to carry out the required service. The Client’s information will not be used for any purpose outside of Phase One Therapy and the service that you have requested or currently receiving.  Where reasonable and practicable to do so, Phase One Therapy will collect Personal Information only from the client or their nominated next of kin.  However, in some circumstances information may be provided by third parties. In such a case Phase One Therapy will take reasonable steps to ensure that the Client is made aware of the information provided by the third party.

Consent and Confidentiality.

By using this website and the Client giving information to Phase One Therapy, the Client has indicated consent. Phase One Therapy aims to use personal information responsibly and ethically.

Phase One Therapy is sometimes required to share the Client personal information such as:

  • With third parties for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with APPs and this policy
  • With other healthcare providers (e.g. In referral letters)
  • When it is required or authorised by law (e.g. court subpoenas)
  • When it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent
  • To assist in locating a missing person
  • To establish, exercise or defend an equitable claim
  • For the purpose of confidential dispute resolution process

Other than providing allied health services or as otherwise described in this policy, Phase One Therapy will not share personal information with any third party without the Client’s consent.

Access to Your Personal Information.

A Client can request access to their personal information at any time and may request to update any information or change any information that is incorrect. A Client may access the Personal Information held and to update and/or correct it, subject to certain exceptions. If a Client wishes to access their Personal Information, please contact Phase One Therapy in writing. Phase One Therapy will not charge any fee for access request, but may charge an administrative fee for providing a copy of the Client’s Personal Information.  In order to protect the Client’s Personal Information Phase One Therapy may require identification before releasing the requested information.

Information about other people.

If information is provided by a third party, they must ensure they are entitled to disclose that information to Phase One Therapy and that they have fully complied with the Privacy Act 1988 in relation to the collection, use or disclosure of that information.

Security of information. 

Phase One Therapy takes security of Clients personal and business information seriously. All reasonable steps are taken to ensure that all information is treated confidentially, kept secure and protected against unauthorised use and is maintained only for the purpose for which it is intended. Client’s Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorised access, modification or disclosure. When the Client’s Personal Information is no longer needed for the purpose for which it was obtained, Phase One Therapy will take reasonable steps to destroy or permanently de-identify a Client’s Personal Information. However, most of the Personal Information is or will be stored in Client files which will be kept by Phase One Therapy for a minimum of 7 years.

Use of artificial intelligence (AI)

Phase One Therapy uses an AI scribe tool to support clinicians take notes during their consultations/ session with the Client or to complete clinical notes or report writing. The AI scribe may use audio listening technology during the session to generate a clinical note for the Client. The practice AI scribe service is Heidi Health or Everbility.

Heidi Health and Everbility:

  • Does not share information outside of Australia
  • Destroys any audio data once the transcription is complete.
  • Retains sensitive, personal identifying information as part of the transcription if integrated with Best Practice.

Phase One Therapy

Communication Policy as if 05/01/2025

There are multiple ways in which Phase One Therapy communicates with Clients and third parties – face to face, telephone, or email are the most common.

Telephone – Clients can contact the main Phase One Therapy number between 9:00am and 3:30pm Monday to Thursday. Contracted Clinicians have varied hours of work and may contacted as per their contracted hours. The admin staff or Clinicians may use a triage system to assess the urgency of the Client’s needs and concerns. Please note, it is often not possible to speak directly to the treating clinician, please leave a voice message in these circumstances.  

Email – Email is not a secure form of communication. If the Client does choose to contact Phase One Therapy via email, this is considered as Client’s consent for Phase One Therapy staff to reply via email. Phase One Therapy endeavour to respond to email messages within 24 hours.

SMS – SMS messages are sent to remind Client’s of scheduled appointments. Please let Phase One Therapy know the Client’s number has changed or they do not wish to be contacted via SMS.

When correspondence is received from other healthcare providers, it is directly imported into the Client’s file. This includes emails, quotes and specialist letters.

Cancellation Policy as if 05/01/2025

A minimum of 24 hours’ notice if the Client can’t make the scheduled appointment.  If notice of cancellation is not provided within this timeframe of the appointment, it may attract a cancellation fee of 100% of the scheduled therapy service fee.  Should a Client travel to an appointment and the Client is not available or in not attendance at their home, the travel fee is also charged. Continued cancellations could lead to appointment removal.
