What is the process?

The process typically begins with a referral from the client, hospital, case manager, or a family member. If possible, the OT may conduct an assessment while the patient is still in the hospital. This may include aspects such as mobility, self-care, cognition or any assistive devices that may be required upon leaving hospital. The OT may conduct a home visit to evaluate the home environment. With input from other stakeholders, a plan will be formulated for any equipment or modifications required, frequency of ongoing OT input, and the provision of education to the individual and family.
